Guideline for Endorsement on Passport ::
   (Add child / children (Age under 12 years) on Passport OR Corrections on Passport)
The following documents are required to be submitted :

  1. Please duly fill-in prescribed form (form can be downloaded from our website). All section must be filled in with correct and appropriate information.

  2. Please Type or Write in Block letters; illegibility may cause delay in the process or sometimes no process at all.

  3. Form to be filled and signed by father or mother of the child and sent back to us with mother or father's passport, provided that he/she are Bangladesh national.

  4. Birth Certificate of child/children, where the names of both father and mother are mentioned.

  5. Two (2) copies stamp size photos (2.5 cm X 2.6 cm) of the child and One (1) copies passport size photos of the applicant (mother or father).

  6. Physical appearance of the child is required.

  7. All fees should be payable to “Bangladesh High Commission”. High Commission will not be held responsible if you have sent any bank draft or postal order blank or in any other’s name.

  8. Please note that your fees/charges vary according to your choice of service priority ( Regular or Urgent).

  9. If you are sending your application by mail, please use the guaranteed or registered or express mail. Ordinary mail sometimes gets lost or occurs delay. Please also enclose a self-addressed envelope with sufficient postal stamps to return applicant's passport by registered or guaranteed or express post.   

  10. We do not accept application by Courier (FedEx, DHL, etc.).

  11. Please sign the application with Place and Date.

    Please note:Incomplete form will not be processed and the High Commission will not be held responsible for such application.
